How it works
Three steps to close the loop.

1.Our volunteers sign up patients on the app.
Trained volunteers and clinic care coordinators get patients onboarded in minutes, or patients can sign themselves up.

2.The app guides them to eligible services.
Lucero matches each patient with the local services they qualify for, from food banks to utility help, and shows them where to go.

3.Our volunteers check in until they get help.
Volunteers and care coordinators follow up and patients self-report, so every referral closes the loop.
